Using P6MyCarePlan App Summary
The P6MyCarePlan App enables the organisation / operators to:
- Upload photos and documents to be made available to the App for residents/clients or their families or representatives to view and communicate about
- Enter Events / Tasks in the calendar for the resident/client to view via the App
- Determine which of the 29 ‘MyCare’ forms and questions residents/clients can access or write answers in
- View details entered in the various 29 ‘MyCare’ forms’ content via the Platinum 6 program, to then act on or refer as required
The P6MyCarePlan App enables the resident/client to:
- Contribute information to their own care plan by entering their own preferences and information via the 29 mini ‘MyCare’ forms
- Access their Summary Care Plan which is based on their answers and staff contributions
- View the Tasks/Events that have been entered via the Platinum program, related to themselves, to know what is coming up
- Communicate needs to staff by entering feedback information
- Take photos and ad them into the App from their own tablet or phone where the App is loaded, so that staff can view them instantly in the program they will have installed on their computer
- View photos and documents loaded into Platinum 6 that have been made available to the App
